As a benefit to our Polar Caps and West Side Hockey Community we have worked with the Mobile Vaccine Clinic to bring the mobile vaccine clinic to MIA on WEDNESDAY, OCTOBER 26, 2022 from 6:00 - 9:00 p.m. This is a clinic that is supported by and accepted by all local health plans that provides vaccines to anyone over age 5. The vaccine clinic will provide vaccines to ANYONE OVER AGE 5, not just registered Polar Caps players. This is an excellent opportunity for you to get your family vaccinated at a time and location that is hopefully more convenient for our families. The following vaccines will be offered:
Pfizer COVID vaccine 5-11 years old
Pfizer COVID 12+ first or second shots
Pfizer COVID Bivalent booster for 12+ years
HPV
Flu
Meningococcal
Tdap (tetanus)
Pre-registration is now closed.
You may register on Wednesday if you want a vaccine, and we will give as many vaccines as we have on hand and try to be as timely as we can based on the number of vaccinators that we have available.

The vaccine clinic asks everyone to sign up ahead of time for their vaccines so that they can ensure that they bring an adequate supply. It also enables their staff to confirm that you are eligible for the vaccine and if you are eligible for more than just what you signed up for they may reach out to see if you want additional shots that day. All vaccines given by the clinic are entered into the Wisconsin Immunization Registry (WIR) and your primary care provider will be able to access all the information about these vaccines. If we have more demand for vaccines than we initially have slots, we will find more vaccinators and open more slots!

WE NEED VOLUNTEERS to make this event a success. THIS WILL COUNT FOR DIBS HOURS! So, if you don’t love working the concession stand, come help with the vaccine clinic! You DO NOT have to have a healthcare background to volunteer, although only those with healthcare backgrounds can administer vaccines. All volunteers will have 2 hour shifts from either 5:30-7:30 or 7-9. Vaccinators are required to take an at home training module that should take 30 minutes or less to complete.
